Qualifications: High School Diploma or equivalent, required.
Associate Degree preferred.
Experience in social services, healthcare, or related field preferred Experience in maternal/child health preferred.
Licenses/Certification/Registration: Certification as a Community Health Worker (CHW-C); or ability to obtain within one year of employment, required.
Job Knowledge: Knowledge of and shared lived experience with the community served.
Knowledge maternal/child health preferred.
Competence in widely agreed-upon principles of client-centered practice: empathic, strengths-based, health-literate, trauma-informed, stigma-free care and services.
Understands fundamentals of evidence-based behavior change, and can support and build on client needs, goals, strengths, and abilities to work towards goals.
Is fluent in how social determinants of health shape health and health equity, as well as the relationship of health to social justice.
Job Summary: The Family Navigator 1 is a frontline health worker who is a trusted member of the community, and has a unique understanding of the community served.
Navigators promote efforts to reduce maternal and child health disparities by identifying and navigating access to a wide range of social and material resources available for perinatal moms.
Navigators work closely with mothers, their natural supports, social service agencies, and medical and behavioral health providers to develop and coordinate a care plan that is culturally/linguistically appropriate, trauma-informed, and strength-based.
They help mom’s access services that promote health and health literacy, reduce barriers or lack of resources, and support the uptake of home visiting services and breastfeeding supports that utilize best practices for imparting parenting skills and infant care.
Skills & Abilities: Ability to communicate easily with others; including demonstrating active listening skills, and communicating with people from different backgrounds, life experiences, and cultures.
Employs communication that is inclusive, person-first, and culturally/linguistically/developmentally appropriate.
Ability to identify and navigate the social service systems and advocate for others.
Ability to work independently and as part of a team.
Demonstrates healthy boundaries.
Utilizes person-centered approaches for helping people understand and feel empowered to address health risks for themselves or families.
Demonstrated response to stressful situations with calmness and professionalism.
Essentail Duties: Makes contact with perinatal moms at the various clinic sites as delineated by the intervention protocol(s) - PMS-Questa Health Center, Union County General Hospital, Holy Cross Medical Center and Miner’s Colfax General Hospital.
Encourages engagement of moms in a comprehensive, evidence-based, social service needs assessment and related referral and navigation interventions to support access to services.
Conducts assessment of social, material, and healthcare needs to collaboratively (with mom) develop a referral and navigation plan of care to help mom’s identify priorities, define goals, and overcome barriers to enjoy a healthy pregnancy.
Encourages the participation in perinatal home visiting programs, and breastfeeding supports.
Explains the benefits of various referrals, services, programs and their eligibility criteria to problem solves barriers to accessing those services.
Build a trusting partnership with moms, their medical provider, social service providers, family and other social supports (as needed).
Maintain a caseload of moms to ensure comprehensive access to indicated services.
Recognize and promote the mom’s strengths, interests, readiness, willingness, and ability to participate in accessing services.
Provide stigma-free, trauma-informed, health literate, and culturally/linguistically/developmentally appropriate care and services to moms and their identified natural supports.
Maintains confidentiality of medical records and personal information in compliance with HIPAA, both electronic and paper records.
Identifies risk factors and safety issues and educates, informs and intervenes as necessary following guidelines and protocol of medical or social service providers.
Actively participates in clinical team meetings, case conferences, trainings, networks, grand rounds, continuing education, and conferences to maintain or improve quality services and delivery systems.
Encourages moms to participate in continuous QI activities of the ROAMS health systems.
