POSITION SUMMARY: The Case Management supervisor is responsible for supporting the case management team under the supervision of the Director of Case Management.
The role requires the employee to work closely with Nursing Leadership and the Hospitalist Group to develop, implement, and maintain low length of stay and ultimately to enhance the quality of care and achieve a high level of patient and provider satisfaction.
This individual, in collaboration with the director, assists in the daily operations of Case Management Department and work flows/processes of the Case Management department.
This individual ensures delivery of care, productivity and timeliness.
Ensures staff is informed of changes impacting their work by communicating updates, policy revision, performance outcomes and significant events.
This role is intended to be split between taking case load on the units and doing administrative work off the floor; the balance of those tasks will be based on need and must remain flexible to departmental need.
This role is not intended to have any supervision of other team members in the department.
The management of interpersonal dynamics, behavior or unsatisfactory work will be the responsibility of the Director of case management.
Case Management supervisor assumes all responsibilities in absence of Director of Case Management.
Requirements
MINIMUM QUALIFICATIONS:
EDUCATION: Graduate of an accredited school of nursing, BSN preferred OR Masters Degree in Social Work (MSW) required
CERTIFICATION/LICENSES: Licensed as a Registered Nurse to practice nursing in the state of New Mexico.
Meets contact hour requirements for licensure, including all state required courses.
OR Current licensure as an LMSW/LISW/LCSW in New Mexico.
Case Management certification highly desirable - ACM (Accredited Case Manager), CCM (Certified Case Manager, RN-BC (Board Certified in Case Management)
SKILLS: Strong leadership skills; experience in facilitating interdisciplinary team collaboration.
Evidence of such experience should include:

Strong analytical skills
Ability to problem solve; ability to appropriately confront issues
Ability to motivate others as individuals and as a team
Ability to plan, organize, direct, and manage the activities of others
Ability to effectively run meetings and to negotiate deadlines
Ability to communicate with a wide variety of audiences
Experience in dealing with physicians on an administrative level
Ability to work collaboratively with medical staff members and leadership
EXPERIENCE: Minimum of 2 years of case management experience.
Managerial experience preferred.
NATURE OF SUPERVISON:
-Responsible to: The Director of Case Management and Chief Financial Officer
ENVIRONMENT:
General office environment and general hospital department/nursing units (100% of day).

PHYSICAL REQUIREMENTS: Subject to stressful professional relationships.
Working hours will vary, with flexibility, due to unexpected changes in schedule and emergencies.
Requires the ability to speak, listen, develop and communicate written materials.
